UHNM first started using Recap Health to share healthcare information with heart failure patients, on discharge from in-patient care.

An independent evaluation, funded by NHS England, has demonstrated that patients using Recap Health together with Flo, another digital tool, are 42% less likely to be readmitted over 6 months than similarly aged patients. Delivering annual savings to UHNM of up to £1.5 million per year.

Evaluation highlights

  • Hospital all cause readmissions rate of 52% at 6 months post discharge compared to usual care group rate of 94%.

  • Equates to relative risk reduction of 42% and annual savings of £1.5 million per year for UHNM.

  • A&E all cause readmissions rate is 34% at 6 months post discharge compared to usual care group rate of 100%. Equates to relative risk reduction of 66%.

  • NHS savings of £50 million a year if applied to the 80,000 patients admitted with heart failure annually.

  • 77% of patients reported a significant increase in self-efficacy scores using KCCQ at 3 months.
